Impaired aerobic capacity, also known as impaired endurance, is a common patient impairment that can limit participation in functional, occupational, and recreational activities. Even functional tasks that require only a few minutes can be limited by aerobic capacity. Older adults are particularly vulnerable to impaired aerobic capacity due to anatomic and physiological changes that occur with aging, greater propensity for sedentary behaviors, and greater risk for disease processes that limit the oxygen transport system.1 In addition, aerobic capacity is directly influenced by the habitual activity pattern of an individual, which may vary across individuals from total inactivity to frequent and intense activity. Any factors that limit habitual physical activity, such as illness, injury, and or travel, will cause adaptations that diminish aerobic capacity. Conversely, any factors that promote habitual physical activity, such as intentional exercise, yard work, and occupation-related physical tasks, will result in adaptations that improve aerobic capacity. In older adults, many physiological, pathological, and psychosocial factors can contribute to restricted physical activity. Figure 12-1 depicts the persistent vicious cycle that can be created when sedentary behaviors, chronic disease, and functional dependency interact.2 This chapter will provide an overview of causes and factors contributing to impaired aerobic capacity in older adults and describes physical therapist patient management (examination, evaluation, diagnosis, and interventions) to address decreased endurance and its impact on function.

You're twice as fit if the muscle can consume all 100 molecules than if it can only consume 50. In terms of performance in this scenario, you'll have more endurance because your muscles won't run out of oxygen as quickly.
Recent research showed that wearing a fitness-tracking wristband (the FitBit One) did help overweight postmenopausal women increase their activity levels by nearly 40 minutes (and 789 steps) a week.2 Wearing a pedometer did not have such an effect. However, if you're committed to making your 10,000 steps a day, does that mean you're on your way to becoming physically fit?

As you get started toward the recommended 30 minutes of moderately intense aerobic exercise five days per week, aim to exercise at a level that just lets you keep up a conversation during the activity. If you can get out three or four sentences in a row without gasping for air, it’s a sign that you’re maintaining an intensity that is truly aerobic, meaning aerobic metabolism is supplying the vast majority of your body’s energy, Jonesco says.

It all starts with breathing. The average healthy adult inhales and exhales about 7 to 8 liters of air per minute. Once you fill your lungs, the oxygen in the air (air contains approximately 20% oxygen) is filtered through small branches of tubes (called bronchioles) until it reaches the alveoli. The alveoli are microscopic sacs where oxygen diffuses (enters) into the blood. From there, it's a beeline direct to the heart.

Better memory and cognitive function. A clinical trial of older adults in Japan published in the Journal of the American Geriatrics Society in 2015 found that after 12 weeks, men and women in a prescribed daily walking exercise group had significantly greater improvements in memory and executive function (the ability to pay focused attention, to switch among various tasks, and to hold multiple items in working memory) compared with those in a control group who were told just to carry on with their usual daily routine.

If you’re not as fit as you’d like to be, walking “can help tremendously,” says Matheny. He calls walking “a great entry point for a lot of people” and says it’s a “key foundation to have in place for getting in shape.” And yes, this “counts” as cardio. “Any physical activity that elevates your heart rate above its normal resting rate can be considered cardio,” says Matheny.
For weight loss, gradually work up to 45 minutes or longer at moderate to vigorous intensity five to six days a week, allowing for at least one day of rest a week. Vigorous intensity refers to an activity that will have your heart beating quite a bit more than moderate intensity workouts, and your breathing will be harder so saying more than a few words will be difficult.
Add strengthening exercises to your walking workout to build muscle. Even though strength training does not burn considerable calories, it replaces your fat with lean muscle mass. Your body works harder to sustain your muscle mass, raising your resting metabolism so you burn more calories throughout the day. Invest in light hand weights or wrist weights and pump your hands as you walk. Build lower body muscles by lifting your knees high during part of your walk. Stop every five minutes and do a series of squats or lunges.
Walking helps to improve your heart health. Irish scientists have reported that walking is the best exercise for sedentary individuals, especially adults, to reduce the risk of heart and cardiovascular diseases (2).In another study published in the Journal of American Geriatrics Society, scientists confirmed that men and women of 65 years of age or older, who walked for at least 4 hours every week, were at less risk of cardiovascular disease (3). So, make sure to walk for 4 hours or more a week to keep heart disease, cardiovascular disease, and stroke at bay.
Your morning cup of java could actually aid your weight loss efforts. According to the results of a 1990 study in the American Journal of Clinical Nutrition, caffeine consumption can increase calorie burn. A second study, published in a 1994 edition of the International Journal of Obesity, found that the consumption of 200 milligrams of caffeine increased calorie burn by 6.7 percent during a three hour period.
1. Walk as much as you can. The University of Warwick study compared people with at least one sign of metabolic syndrome—a group of risk factors (high blood pressure, fat around the waist, high blood sugar, and high triglycerides and cholesterol) for heart disease—to those with no risk factors. They found that those who got the least activity had the most risk factors, and those who walked the most—accumulating at least 15,000 steps per day—had healthy BMIs, smaller waists, lower cholesterol and blood pressure, and better blood sugar control.
Triglycerides are a common form of fat that we digest. Triglycerides are the main ingredient in animal fats and vegetable oils. Elevated levels of triglycerides are a risk factor for heart disease, heart attack, stroke, fatty liver disease, and pancreatitis. Elevated levels of triglycerides are also associated with diseases like diabetes, kidney disease, and medications (for example, diuretics, birth control pills, and beta blockers). Dietary changes, and medication if necessary can help lower triglyceride blood levels.

A higher percentage of fat is burned during aerobic exercise than during anaerobic exercise. Here's why. Fat is denser than carbohydrate (fat has nine calories per gram and carbohydrate has four), and so it takes more oxygen to burn it. During aerobic exercise, more oxygen is delivered to the muscles than during anaerobic exercise, and so it follows that a higher percentage of fat is burned during aerobic exercise when more oxygen is available. When less oxygen is present, like during anaerobic exercise, a higher percentage of carbohydrate is burned. 

The statistics are impressive: The American Diabetes Association says walking lowers your blood sugar levels and your overall risk for diabetes. Researchers at the University of Boulder Colorado and the University of Tennessee found that regular walking lowered blood pressure by as much as 11 points and may reduce the risk of stroke by 20% to 40%. One of the most cited studies on walking and health, published in the New England Journal of Medicine in 2002, found that those who walked enough to meet physical activity guidelines (30 or more minutes of moderate activity on 5 or more days per week) had a 30% lower risk of cardiovascular disease, compared with those who did not walk regularly. "The physical benefits of walking are well documented," says Scott Danberg, director of fitness at Pritikin Longevity Center + Spa in Miami. With impressive results like these, there's a good chance you'll get a pat on the back from your doc at your next checkup.

British physiologist, Archibald Hill introduced the concepts of maximal oxygen uptake and oxygen debt in 1922.[4][5] German physician, Otto Meyerhof and Hill shared the 1922 Nobel Prize in Physiology or Medicine for their independent work related to muscle energy metabolism.[6] Building on this work, scientists began measuring oxygen consumption during exercise. Notable contributions were made by Henry Taylor at the University of Minnesota, Scandinavian scientists Per-Olof Åstrand and Bengt Saltin in the 1950s and 60s, the Harvard Fatigue Laboratory, German universities, and the Copenhagen Muscle Research Centre among others.[7][8]
Figure 78 shows the distribution of total aerobic (T. aerobic) organisms, aerobic bacteria, fungi, facultative anaerobic (Fa. aerobic) bacteria, and total anaerobic (T. anaerobic) bacteria in unplowed and plowed soil (Linn and Doran, 1984). This study is based on the hypothesis that plowing may increase oxygen and moisture, thus increasing that aerobic capacity and decreasing the ratio of population in nonplowed (PnP) soil/population in plowed (PP) soil for soil samples. In samples collected from 0 to 75 mm depth, the PnP/PP ratio ranged from 1.27 to 1.35, indicating that plowing reduced bacterial (aerobic or anaerobic) populations in top soil samples, possibly due to the lost moisture. In samples collected from deep soil, the PnP/PP ratio ranged from 0.6 to 0.8 for aerobic microbes, while the ratio ranged from 0.9 to 1.1 for anaerobic microbes. This suggests that although plowing significantly increased aerobic microbial population and/or decreased anaerobic populations, the surface microbes were more susceptible to plowing than were deep-soil microbes (Li et al., 2014a).

Whether you're feeling stuck at work or you've been searching for a solution to a tricky problem, research shows it's a good idea to get moving: According to a 2014 study in the Journal of Experimental Psychology, Learning, Memory, and Cognition, going for a walk can spark creativity. "Researchers administered creative-thinking tests to subjects while seated and while walking and found that the walkers thought more creatively than the sitters," says Dr. Jampolis.
After World War II, non-organized, individualistic, health-oriented physical and recreational activities, such as jogging, began to become popular.[9] The Royal Canadian Air Force Exercise Plans, developed by Dr. Bill Orban and published in 1961, helped to launch modern fitness culture.[10][11] There was a running boom in the 1970s, inspired by the Olympics.[12]
You can use the cue "warm and slightly out of breath" to gauge your aerobic activity, or you can get more precise and use heart rate. I recommend the heart rate reserve method for calculating a target heart rate. The formula and an example of the method for someone 27 years old, assuming a resting heart rate of 70 beats per minute (bpm), and a training range of 70%, may be found below. Aerobic exercise falls in the range from 40% to 85%. You can plug in your own values to find your aerobic range.
How to measure progress: Other than simply feeling great and watching the scale, you can actually measure what walking is doing for your body. Before you begin your activity program, have your doctor check your cholesterol, blood pressure, glucose (high levels can be a sign of early or undiagnosed diabetes), and your body mass index (BMI). BMI is a number that reflects your height-to-weight ratio (simply take your weight in pounds, multiply by 703 and divide by your height in inches squared -- keep this number under 25 for optimal health).
In normal daily activity, adults cover about 2-3 miles. About 2,000 steps equal a mile. To make walking a beneficial activity, you would need to come up with at least another 4,000 steps in a day. You can find ways to add steps here and there, such as walking farther from parking lots, taking stairs when available instead of escalators and elevators, walking the long way to get somewhere in your office building, walking your children to school, having a walk/talk meeting instead of sitting down in a conference room, or planning a short walk around the block as a break.
One final note. Spin class is interval training. It's done at gyms on special spin cycles with an instructor who barks out orders to increase the intensity and then slow down to catch your breath. It's addictive, and people who do it regularly swear by it. You should already be doing some aerobic exercise and be reasonably conditioned before you try it, but I recommend it if you're looking for one of the toughest workouts around.
You could increase your calorie burn by drizzling some hot sauce on your food. Hot sauce is made from hot peppers, which contain a spice called capsaicin. According to a 2012 study in the journal Chemical Senses, capsaicin increases both calorie burn and fat burn. Use hot sauce to add some flavor to a chicken breast for a healthy dinner, or mix in some hot sauce to spice up your scrambled eggs.
These changes yield major heart health benefits, with research published in the American Journal of Cardiology showing that aerobic training is the most efficient method of exercise for improving cardiovascular health. If you stand on one leg and swing the other backwards and forwards, you can see this immediately: your forward swing will be higher than your backward, and on the backswing you’ll want to bend your knee. But when the hamstring is properly “recruited” – this is what it’s called, when a movement activates a muscle – it has huge propulsive power, as well as opening up the world in which your glutes (butt muscles) can also do some of the work.

Relax your shoulders and bend your elbows. Bring your arms up to a 90-degree angle, but no more. Straight arms can lead to swelling or numbness of the fingers. Swing your arms naturally with each step, and should be bent at the elbow at a 90? angle. Your elbows should be close to the torso, with the hands going no higher than the center of the chest on the forward swing, or past the back of the hip on the back swing. Faster arms will make faster feet.
There are two physical activity guidelines in the Unites States. The first, the Surgeon General's Report on Physical Activity and Health, is a lifestyle recommendation. That is, you can modify it to fit into your daily routine and activities of daily living. The recommendation is that all adults should accumulate 30 minutes of moderate-intensity activity on most, if not all days of the week. The key words are "accumulate" and "moderate-intensity." Accumulate means that you can do 10-15 minutes at a time and repeat that a couple of times throughout the day; for example, 10 minutes in the morning, 10 minutes at lunch, and 10 minutes around dinner. Moderate intensity is equivalent to feeling "warm and slightly out of breath" when you do it. Recently there has been some controversy about the effectiveness of this guideline and its benefits. At the moment the recommendation stands, but we may hear more about it in the not-too-distant future.

All that oxygen being pumped by the blood is important. You may be familiar with the term "oxygen consumption." In science, it's labeled VO2, or volume of oxygen consumed. It's the amount of oxygen the muscles extract, or consume from the blood, and it's expressed as ml/kg/minute (milliliters per kilogram of body weight). Muscles are like engines that run on fuel (just like an automobile that runs on fuel); only our muscles use fat and carbohydrates instead of gasoline. Oxygen is a key player because, once inside the muscle, it's used to burn fat and carbohydrate for fuel to keep our engines running. The more efficient our muscles are at consuming oxygen, the more fuel we can burn, the more fit we are, and the longer we can exercise.
